• Candidates must be between 12-14 years of age.

• The questionnaire form must be filled out and returned no later than May 15th, 2010. Please ensure you indicate your age on your submission.They can be dropped off at the clinic or sent electronically to; lisa.allandale.vet@on.aibn.com
